Get a Registration Form
Call or visit reception to collect a New Patient Registration Form, or request one by email at info@russellcentrefamilypractice.com.
Submit Your Documents
Return the completed form in person or by post along with the required identification documents.
Book Your First Appointment
Once registered, book your first appointment online via Pippo or by calling us directly.
What to Bring to Your First Appointment
- Photo ID (passport or driving licence)
- Proof of address (utility bill or bank statement dated within 3 months)
- Medical card or GP visit card (if applicable)
- List of current medications
- Any recent test results or hospital letters
- Previous GP's contact details (if transferring)
Medical Card & GP Visit Card
If you hold a valid HSE Medical Card or GP Visit Card, most GP services are free of charge. We are a registered GMS practice on the HSE GP panel. If you are unsure of your eligibility, visit hse.ie or call the HSE on 1800 700 700.
We can assist with applications — ask at reception and our team will be happy to help guide you through the process.
