A chronic disease is a long-term health condition requiring ongoing treatment and management. Our practice is enrolled in the HSE Chronic Disease Management Programme, which provides structured care designed to improve the health and quality of life of patients with certain long-term conditions.

Conditions Covered

The HSE Chronic Disease Management Programme covers the following conditions:

  • Type 2 Diabetes
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Cardiovascular disease (heart failure, angina, stroke, atrial fibrillation)
  • Asthma
  • Hypertension (high blood pressure)

Eligible patients must be aged 18 or over and hold a medical card or GP visit card. The programme provides two structured reviews per year at no additional cost, including practice nurse visits and blood tests.

What Your Care Plan Includes

Each patient enrolled in the programme receives personalised, structured support:

  • Structured GP and nurse reviews twice yearly
  • Personalised care plan tailored to your needs and goals
  • Medication review by your GP
  • Self-management education and support
  • Early detection of new conditions or complications
  • Community-based care close to home

Your Involvement

Participation in the programme is entirely voluntary. You can join or leave at any time by speaking with your GP. Your care is designed around your individual needs and health goals, and we encourage active partnership between you and your healthcare team.

Your personal information is stored in line with current data protection regulations and the HSE's data governance policies.
